A bit of feedback about the questionnaire:
1) The lack of a "no" answer on the first page of questions is a bit confusing.
2) The rating scales on questions 14 and 17 are counterintuitive, that is, one would expect 1 to be "not very often" and 10 to be "very often", instead of the other way round. Expect a fair number of data entry errors for these questions, since people do not have the tendency to read the scale meanings question-by-question.
3) Not that it's a major issue by-and-large, but the questionnaire does not seem to accommodate for Civil Service Card holders, whose fees are covered by the government.
