A bit of feedback about the questionnaire:
1) The lack of a "no" answer on the first page of questions is a bit confusing.
2) The rating scales on questions 14 and 17 are counterintuitive, that is, one would expect 1 to be "not very often" and 10 to be "very often", instead of the other way round. Expect a fair number of data entry errors for these questions, since people do not have the tendency to read the scale meanings question-by-question.
3) Not that it's a major issue by-and-large, but the questionnaire does not seem to accommodate for Civil Service Card holders, whose fees are covered by the government. -

One major problem I found with the questionnaire was that the author should've put in an 'Others' field in the choice of hospitals a patient chose to have surgery in! (e.g. private hospitals) -

i did it but the survey's a mess...
i don't know what the author wants in the 2nd page so i anyhow write something...
frankly, healthcare in singapore... strange thing.. true story (what is going on right now with me).. the orthopaedic fella who checked me out for my slipped disc, kinda snorted that a chiropractor could have actually helped me.. heck! 15 minutes on the table, i was no in pain anymore!
now, if they were given access or did not learn to deride other alternative forms, i may not have had to go through an operation in early 1997 for the original injury!!
still in the face of evidence that an alternative worked, they still deride it..
i think sometimes their hubris gets in the way of healing the sick... -
Actually, what's the purpose of the survey? Is the Ministry of Health planning a fee hike?

A friend of mine sent me this questionnaire in an email - another friend of hers is working in SATA & wanted about 100-150 responses. Don't know for what though.

Can't resist this one, tare:
- Appendix surgery: self-explanatory.
- Colonoscopy: a fibre-optic cable down your throat or up your arse, depending on which part of your colon they want to check out.
- Child delivery (Normal delivery): POP! goes the weasel!
- Child delivery (Caeserian): they slice you up and take the kid out. Salad, anyone?
- Haemorrhoid surgery: your arse has gone from innie to outie, and they're going to turn it back into an innie.
- Kidney and urinary tract conditions (Cytoscopy) - working on the other part of the plumbing.
- Lumps and bumps removal: self-explanatory, but does not include zits.
Now go finish the questionnaire!
