A very very good question!
1) Gays/Lesbians are not transexuals
Gays/Lesbians = Mentally and physically men/women
Transgender = Mentally men/women but physically women/men
Transexuals (thx to heng@ for reminding) = operation to physically align physical gender with mental gender
2) Gays are not necessary more feminine (even though there might be a correlation).. Just like there are straights who are not 100% masculine.. There are 2 continuum we are talking about here..
Homosexuals - Bisexuals - Heterosexuals
Feminine - Androgenous - Masculine
Being in one category in the first continuum does not necessitate him/her to fall automatically into it's (seemingly) associated category.. You can have feminine straights (e.g., michel gondry, baz, etc).. You can have masculine gays (e.g., me!

3) You can't tell gays from straights unless they are willing to out to you.. (Minus those who intentionally dress in certain way or behave in certain manner to out themselves)
