For B, the 22Na will become 22Mg instead if it is a process of that sort.

It means the species generated is a unipositive cation. Your options should read :
A) 11C --> 12C+
B) 22Na--> 22Ne+
C) 32P --> 31P+
D) 40K --> 40Ca+
